Middle School Band Camp
June 15-20, 2008
for students entering 7th and 8th grades next Fall
Registration Deadline: June 6, 2008


Two Ways to Register:

  1. Register On-line & Pay with Credit Card
  2. Register by Mail & Pay with Check

  1. Print and complete the Authorization for Medical Treatment form*.

  2. Mail the completed Authorization for Medical Treatment form to the address listed below:

    Attn: Donna Cornwall
    Box 13043 - SFA Station
    Nacogdoches, Texas 75962-3043

  3. Print out and carefully read:

    2008 Middle School Band Camp Information Sheets*
    SFASU Campus Map*

  4. Go to Band Camps Audition Information. Select and print the appropriate audition music*.

  5. Go to Band Camps: Middle School (in the School of Music BAND CAMP Store).

  6. Select either:

    MS Band Camp: FULL CAMP
    or
    MS Band Camp: COMMUTER

  7. Select your payment method: Visa, MasterCard, Discover, AmEx.

  8. Click ADD TO CART.

  9. Complete the secure on-line registration form, payment info, billing address
    and click SUBMIT.

  10. You will receive an automatic e-mail Registration Confirmation.

  11. You will receive a 2nd e-mail notification once your credit card payment has been processed.



Register by Mail & Pay with Check

  1. Print and complete the 2008 Band Camp Application form*.

  2. Mail the Application and your check ($420 full payment or $200 deposit) to the address listed below:

  3. Print out and carefully read:

    2008 Middle School Band Camp Information Sheets*
    SFASU Campus Map*

  4. Go to Band Camps Audition Information.
    Select and print the appropriate audition music*.
