Two Ways to Register:

1. Print and complete the Authorization for Medical Treatment form*.
   
   
2. Mail the completed Authorization for Medical Treatment form to the address listed below:

3. Print out and carefully read:
   
   2008 High School Band Camp Information Sheets*
   SFASU Campus Map*
   
   
4. Go to Band Camps Audition Information. Select and print the appropriate audition music*.
   
   
5. Go to Band Camps: High School (in the School of Music BAND CAMP Store).
   
   
6. Select either:
   
   High School Band Camp: FULL CAMP
   or
   High School Band Camp: COMMUTER
   
   
7. Select your payment method: Visa, MasterCard, Discover, AmEx.
   
   
8. Click ADD TO CART.
   
   
9. Complete the secure on-line registration form, payment info, billing address and click SUBMIT.
   
   
10. You will receive an automatic e-mail Registration Confirmation.
    
    
11. You will receive a 2nd e-mail notification once your credit card payment has been processed.

1. Print and complete the 2008 Band Camp Application Form*.
   
   
2. Mail the Application and your check ($420 full payment or $200 deposit) to the address listed below:

3. Print out and carefully read:
   
   2008 High School Band Camp Information Sheets*
   SFASU Map*
   
   
4. Go to Band Camps Audition Information. Select and print the appropriate audition music.
